Students are not to have prescription or over-the-counter medications in their possession on school transportation, during the school day or during school sponsored events. Students must take medication only in the presence of the licensed school nurse, medication assistant or parent/guardian. In limited cases with asthma or diabetes a student may be authorized by their prescriber to self-medicate. A medication patch prescribed by a licensed medical professional may be worn during the school day. Please alert the school nurse if your student will be wearing a medication patch.
Prescription medication will only be administered if a School Medication Prescriber/Parent Authorization Form is properly completed and signed by the licensed prescriber and the parent/guardian.
School Medication Prescriber/Parent Authorization Forms for the over-the-counter medications on the physician approved formulary my be completed and signed by the parent/guardian. All over-the-counter medication must be in new unopened containers and brought to school by an adult. Students are not authorized to transport medication.
